Vulnerable Sector Check (Police Check)

All SGMHA volunteers (coaches, assistant coaches, managers, trainers, and executive) require volunteer screening.

Visit https://www.opp.ca/index.php?id=147 and complete steps 1 to 3

Select "Apply for a Record Check"

MASTER POLICE CHECK - VSC letter

You MUST Download and fill in your NAME and select Bench Position before you attach to application while applying online.

SAVE and upload an electronic version of your completed form to your police record check application when prompted.

When all applicable information and documentation has been provided, hit "submit" and retain your confirmation code (a confirmation email will be sent to the email address provided) 

BEFORE YOU PROCEED TO SCREENING PORTAL - if your VSC is expired or expiring within the 2026/27 season (obtained in 2023), a new one is needed. If your VSC is still valid and does not expire during the 2026/27 season, you will need to sign the online declaration which can be done by filling out the form in Step 3 below and answer "No" to question 8.​​​​​​​​​​​​​​

Your VSC is your responsibility.

IMPORTANT FINAL STEP - Once you receive the completed/approved Police Check form from the OPP, follow instructions below. 

Hockey Eastern Ontario continues with its Vulnerable Sector Check (VSC) process in collaboration with Ontario Hockey Federation and Hockey Northwestern Ontario.

The centralized VSC process streamlines and strengthens background checks for all involved in youth hockey, ensuring that vulnerable sector checks are conducted with greater efficiency. All Team Officials, Third-Party Instructors, and On-Ice Officials over the age of 18 are required to have a VSC.
